I think it is completely up to you. BUT whatever way you do it, just make sure you continue to do it that way to maintain a nice base measurment and maintain consistancy.

I'd go sans-bra, as if you're like me, you have many bras and may not remember to wear the same one for each measurement.

Also, if they're of the push-up variety, you may be losing but that might not be reflected in the measurements due to the firm cup.

I agree with both Linaeve and Minu - consistency is the important point. And if you do it with the bra it is important to remember which one as they all support/push up differently. I have gone done 2 full cup sizes since Christmas so I've had to purchase new bras - do the hold "them" the same as my old bras? God knows!

I usually do it naked or with one of my fitness tops on which usually holds (or flattens) them to the same degree!

For me, it's much easier to measure consistently with a bra on, because the bra squishes it all together and makes it a bit firmer. That makes it easier to get good tape readings. Of course, it wouldn't be quite accurate if the bra you're wearing has a bit of padding, etc.

I get my measurements with full clothing b/c I get measured by a male trainer. Haha. But it is before a work out at the gym so it's just a sports bra - aka no padding.
